We're pleased to announce that Maimonides Medical Center has selected Relevant's care management platform to support its Health Home program!
Maimonides has been at the forefront of developing collaborative, community-based models of care for Brooklyn's Medicaid population, and we look forward to supporting its work as these models evolve over the next several years.
Relevant's focus will be to provide software and services for Brooklyn Health Home, which is managed by Maimonides and serves some 8,000 Medicaid members with chronic physical or behavioral health conditions, social risk factors, or other qualifying criteria.

"The priorities for us are friendly workflows, ease of use, and visibility into the 'big picture,' whether that's helping a care manager prioritize work across a caseload of 50 patients or allowing us to analyze risk across thousands of members," she added.
